Hm, think I solve it with database repair

What sort of Destination is this? Google Drive and relatives are more prone to this, because multiple files having the same name are possible, and Duplicati expects file names to be unique. If it keeps happening, maybe you could run a log-file at log-file-log-level=retry to allow study of how the duplicate names occurs.
